Our knee joint is not only the largest joint in our body but also it is the joint which performs a lot of activities. It is a hinge type of joint formed between Femur(thigh bone) and Tibia(Shinbone) and Patella (knee cap), the ends of these bones are covered with cartilage which keeps the surfaces of bones smooth and protects them from grinding against each other. There are menisci between Femur and Tibia which act as a shock absorber and provide with the cushioning effect

Knee Surgery is a surgical procedure to replace the weight-bearing surfaces of the knee joint to relieve pain and disability. Earlier with conventional approach extensive bone cutting and soft tissue damage used to be there but now with advanced technology like robotic assisted joint replacement as available in Lokmanya hospitals, now the procedure is less complex with better outcomes. Procedure includes:

  • Preparation of the Bone/ Resurfacing of bone
  • Placement of implant/prosthesis
  • Insertion of spacer as substitute of Meniscus
  • If patellar bone is affected then it ia also resurfaced otherwise is kept as it is

With Modern Technology ligaments and muscles and other soft tissues are preserved so there is faster healing and patient satisfaction and acceptance is higher.

It is most commonly performed for osteoarthritis, and also for other knee diseases such as rheumatoid arthritis, psoriatic arthritis, post traumatic arthritis Common Indications for knee Replacement Surgery are are:

  • Severe knee pain or stiffness that limits your everyday activities, including walking, climbing stairs, and getting in and out of chairs. You may find it hard to walk more than a few blocks without significant pain and you may need to use a cane or walker
  • Moderate or severe knee pain while resting, either day or night
  • Chronic knee inflammation and swelling that does not improve with rest or medications
  • Knee deformity — a bowing in or out of your knee
  • Failure to substantially improve with other treatments such as anti-inflammatory medications, lubricating injections, physiotherapy, or other conservative procedures.

Types Of Treatments

Total Knee Replacement (TKR)

It is advised when more than one compartment out of 3 compartments- including Medial compartment, Lateral compartment, and Patellofemoral Compartment -of the knee are damaged.

Most total knee replacement operations involve replacing the joint surface at the end of your thigh bone (femur) and the joint surface at the top of your shin bone (tibia) where the two bones are coming in contact with each other due to worn off cartilage and damaged meniscus, with a Metal or Ceramic prosthesis and a spacer made up of high-density plastic. This separates the two bones again and provides the cushioning effect replicating the natural knee anatomy.

It is one of the most commonly performed surgical procedures all over the world with excellent outcomes which include:

  • Pain relief
  • Correction of deformity
  • Increased mobility
  • Freedom from disability and dependency on others

Once the surgeon has advised the Total Knee Replacement Surgery on the basis of Medical History, Clinical Examination and Radiographic analysis, certain preoperative investigations are advised and after fitness certificate given by the physician and anesthetist surgery is planned. Now with advanced technology as available at Lokmanya Hospitals surgical time is smaller and patient doesn’t require to rest for a long time, also length of stay at the hospital is also less and before discharge patient is made to do all his activities on his own. Postoperative rigorous physiotherapy is also not a major requirement. Basic exercises routine and healthy diet helps patient to recover even faster.

Partial Knee Replacement (UKA)

It is the surgery which is being advised when only one compartment of knee is damaged. Most commonly affected compartment is Medial compartment but any of the 3 compartments- Medial (inner), Lateral (outer) and Patellofemoral (between patella and Femur) Compartments can be affected. Only the affected compartment bone is resurfaced and replaced with Metal or Ceramic prosthesis and high-density plastic material.

The advantages of partial knee replacement over total knee replacement include:

  • Rapid recovery
  • Less pain after surgery
  • Lesser blood loss
  • Lesser length of stay than Total knee replacement

Also, because the bone, cartilage, and ligaments in the healthy parts of the knee are kept intact, many patients report that a unicompartmental knee replacement feels more natural than a total knee replacement. A unicompartmental knee may also bend better.

Partial knee replacement is more technique sensitive and case selection is also very important for the success. After careful clinical and radiographic examination and fitness clearance by physician and anesthetist, surgery planning is done. For Partial knee replacement high grade precision is of utmost importance.
